IMPORTANT INFORMATION FOR STUDENTS LIVING ON CAMPUS
The Office of Health Promotion and Wellness is committed to the advancement of the health and well-being of the University of St. Thomas community.
Starting this Fall 2009, all students who live on campus will be required to have or have record of the following immunizations:
• MENINGOCOCCAL MENINGITIS
• MMR
• Tdap
• HEPATITIS B
• TUBERCULOSIS
Immunization records or Statement of Immunization Exemption forms must be sent to the University HP&W Department no later than Wednesday, September 2, 2009. We recommend that you keep a copy of these forms for your personal records. Upon completion, students or healthcare professionals are required to mail the immunization form to:

If you have not received the required immunizations at the time of move-in, there will be an opportunity to receive them at your own expense. A nurse from University of Texas Health Services will be available on the following dates at Guinan Residence Hall:
Wednesday, August 19th – 9:00am – 1:00pm
Friday, August 21st – 9:00am – 11:00am
These records are filed at the Office of Health Promotion and Wellness and are kept strictly confidential. The information contained in this form is accessible only to the professional staff of the Health Promotion and Wellness Office and will not be released without the written authorization of the student or pursuant to a lawfully issued subpoena.
