Please carefully read the Student Organization Manual for thorough guidelines concerning the forms below. If you have any questions feel free to contact the Office of the Chair.
Note: All funding request and new club request paperwork are due 48 hours before the COC meeting. Forms can be turned in at the COC mailbox located in the Student Activities suite on the first floor of Crooker Center or via email to Marium Nisa at coc@stthom.edu.
All budget update forms are due the first Thursday of every month. Reconciliation forms and post assessment forms are due a week after the event. Forms can be turned in at the COC mailbox located in the Student Activities suite on the first floor of Crooker Center or via email to Maria Lino at coc-treasurer@stthom.edu.
Club constitutions are due no later than Septemeber 30, 2012. Please contact Anthony Pham at coc-vicechair@stthom.edu for more information.
If you are seeking a petty cash slip, check request form, or tax exemption form, please contact Maria Lino at coc-treasurer@stthom.edu.
