How do I request and terminate Phone Service?
A. New Employees.
- Departmental supervisors are responsible for notifying IT that new service or the reassignment of existing service is required. This can be done through the IT Action form provided by the IT Department.
B. Additional Service/Special Services.
- In some cases a user may need an additional line for faxing, RSVP, answering trees, etc. If required, please contact the IT Help Desk at x6900 and give them a detailed description of the service/s desired and they will create a work order for the appropriate parties.
C. Termination of Services.
- For employee termination requirements, an IT action form must be filled out by the departmental supervisor specifying action/s to be taken.
