Forms

The following forms are required by the University of St. Thomas Undergraduate and Graduate Admissions Offices and the International Student Office of all new incoming students. You will need Adobe Reader to be able to read and download these forms.

Application Forms		Descriptions

International Undergraduate Application		Required of all incoming undergraduate level freshmen, transfer and visiting students. Please note that this application must be completed in addition to the regular Undergraduate Admissions Application.

International Graduate Student Information Form		Required of all incoming F-1 Graduate students.

International Student Advisor's Report for F-1 Transfer students		Required of all F-1 students transferring to UST from another U.S. educational institution.

Non-F-1 International Student Form		Required for all new graduate level students holding a Visa other than F-1 (i.e L-1, H1-B, H-4, Asylee, etc).

Proof of Financial Responsibility

A Certificate of Eligibility, also known as a SEVIS I-20 Form, will not be issued until the following required forms are completed and returned to the International Student Office. Please note the following when completing these documents:

1. Monetary funds must cover all expenses incurred during your enrollment at the University of St. Thomas.
2. Both forms must be accompanied by a bank letter.

3. Please see Affidavit for required amount needed for academic year.

Financial Forms		Descriptions

Financial Support Form (F-1 students only)		Required of all F-1 students who will use monetary funds from their own personal accounts to support themselves while attending the University.

Affidavit of Support Form (F-1 students only)		Required of all F-1 students who will receive their monetary funds from their parents or through other sources (i.e. other relatives, U.S. sponsors, or government agencies).
