Important Information for All New Students

Bacterial Meningitis Information:
Effective January 1, 2012, Senate Bill 1107 requires that ALL entering students, including transfer and graduate students, submit evidence that they have been vaccinated against Meningococcal Meningitis.  Vaccination must be within 5 years prior to enrollment, or a student must qualify for one of the two permissible exemptions.

A Student will not be able to register for classes until the appropriate records are on file in the Office of Health Promotion & Wellness.

In accordance with the Center for Disease Control (CDC) recommendations, the bacterial meningitis vaccination must have been received at least after the student’s 16th birthday and within the past 5 years.

  • This information DOES NOT apply to students 30 and older.

Important Information for Students Living on Campus

ALL students who reside in on-campus housing must submit evidence that they have been vaccinated against meningococcal meningitis, along with the following:
  • Had the Meningitis Immunization (MPSV4 or MCV4) AT LEAST after 16th birthday and within the past 5 years
  • Tetanus-Diptheria-Pertussis (Tdap):
    • Primary series plus booster required
    • Must be within last TEN years
  • M.M.R. (Measles, Mumps Rubella)
    • Two doses required or individual vaccine
  • Tuberculosis (copy of skin test)
    • New test required if never received PPD skin test, PPD skin test result was positive, or international student
  • If PPD skin test was positive or if you are an international student, then you must have been tested within the last 12 months

Vaccinations must be administered no fewer than 10 days prior to the student's move-in date. Students will not be permitted to move into on-campus housing until a full 10 days has passed since the date of the vaccination. Students who have been approved for on-campus housing, but who have not received a vaccination in a timely manner will be expected to make alternate arrangements for housing until the full 10 days have elapsed.

Evidence of vaccination must be submitted in one of the following three formats:

  • A UST Immunization Form bearing the signature or stamp of the physician or his/her designee, or public health personnel (must include the month, day, and year the vaccination was administered).
  • An official immunization record generated from a state or local health authority bearing the signature or stamp of the physician or his/her designee, or public health personnel (must include the month, day, and year the vaccination was administered).
  • An official record received from school officials, including a record from another state (must include the month, day, and year the vaccination was administered).

Evidence of declining vaccination must be submitted in one of the following two formats:

  • A UST Bacterial Meningitis Immunization Medical Exemption Affidavit signed by a physician  who is duly registered and licensed to practice medicine in the United States, in which it is stated that, in the physician’s opinion, the vaccination required would be injurious to the health and well-being of the student.
  • An affidavit signed by the student stating that the student declines the vaccination for bacterial meningitis for reasons of conscience including a religious belief. A conscientious exemption form from the Texas Department of State Health Services must be used.
  • A secure online form entitled “Affidavit Request for Exemption from Immunizations for Reasons of Conscience” can be found at the Texas Department of State Health Services website.

  • Please note that the form provided by the University MUST bear the signature or stamp of the physician or his/her designee, or public health personnel (must include the month, day, and year the vaccination was administered).

You can receive all of these vaccinations from your primary care physician. All appropriate vaccinations must be on file in the Office of Health Promotion and Wellness no later than 10 days prior to classes beginning.
