If you are younger than 22 evidence of vaccination must be submitted in one of the following three formats:
- A UST immunization form*
- An official immunization record from a state or local health authority*
- An official record received from state officials, including a record from another state
Whatever you submit must include the month, day and year the vaccination was administered.
*Must be signed or stamped by a certified, licensed physician or his/her designee, or public health personnel.
- Immunization records can be submitted through your MYSTTHOM, using the Immunization Form Upload tile located on your homepage.

NOTE: students will not be able to register for classes or sign up for housing until the appropriate records are on file.
If you are an online student and your courses are not taken in-person, please disregard. Only our Pragmatic Studies students in our AAS program are required to submit their Immunization Records, as they will be attending courses on-campus.
