Below are some forms that will assist you in keeping you and your personal items safe.

This form outlines the guidelines you will need to use if you encounter an armed subject or disruptive individual.
This form will allow you to register your personal property (example: TV, iPhone, etc.) with USTPD. If it is stolen, the police department will have all the necessary information to find it.
Remember: The best way to solve crime is to prevent crime. Do your part in keeping our campus safe. Report all suspicious activity.

Reporting to the Title IX Coordinator will not result in a criminal investigation but will result in an administrative investigation to determine if this policy was violated.If the incident occurred on campus, limited information will be shared with the UST Police for purposes of determining if a Timely Warning Notice should be sent out to the community and for purposes of capturing and counting crime data.
Reporting to the UST Police may initiate a criminal investigation into your complaint.The UST police will also determine if a Timely Warning Notice should be sent out to the community and will capture and count crime date if the offense was reported to have occurred on campus or on a property owned or controlled by the institution.The UST police will report your complaint to the Title IX Coordinator.You can tell the police as much or as little as you would like.
Important Note- The Houston PD may not share the details of your report with UST (however, the campus police must notify the Title IX Coordinator of a report made to their office). If you report to the city police, you should also report to the Title IX Coordinator so on campus accommodations, interim protective measures and resources may be provided, like change in class schedule, housing, work location, or transportation options if you request these and if they are reasonably available.

Mental illness is an important public health problem in itself- about 25% of U.S. adults have mental illness. Mental illness is a condition that impacts a person’s thinking, feeling, or mood, as defined by the National Alliance on Mental Illness. Mental illness may affect a person’s ability to relate to others and function on a daily basis. Mental illnesses include bipolar disorder, post-traumatic stress disorder, depression, and schizophrenia, among others.

Dating and Relationship Violence
Relationship violence is a pattern of behavior in an intimate relationship that is used to establish power and control over another person through fear and intimidation. The National Intimate Partner and Sexual Violence Survey, conducted in 2010 by the CDC found more than 1 in 3 women and 1 in 4 men in the U.S. are survivors of relationship violence in their lifetime.

As defined in Article 56, Texas Code of Criminal Procedure, a victim of a violent crime is someone who: (1) is the victim of the offense of sexual assault, kidnapping, aggravated robbery, trafficking of persons, or injury to a child, elderly individual, or disabled individual or (2) who has suffered personal injury or death as a result of the criminal conduct of another or (3) is the close relative (spouse, parent, brother, sister, or adult child) of a victim; or (4) is the guardian of a victim. Texas also has anti-phishing state laws making it a crime to reference the name, domain address, phone number or any other identifying information of a person without that person's consent, intending to cause the recipient to think the message is truly coming from that person, with the intent to harm or defraud someone.
Identity Theft Laws:
Identity theft laws in most states make it a crime to misuse another person's identifying information -- whether personal or financial. Such data (including social security numbers, credit history, and PIN numbers) is often acquired through
Identity theft is one of the fastest-growing crimes in the nation, robbing its victims of time, money and peace of mind.
Penalties and Sentences:
The penalties for computer crimes are of a wide variety depending on the nature and seriousness of the crime committed. For the breach of computer security (i.e. gaining access to a computer without the consent of the owner), the penalty may range all the way from a "Class B" misdemeanor (up to 180 days in a county jail and/or a fine of up to $2,000) up to a first degree felony (five to 99 years in a state prison and/or a fine of up to $10,000). The factor influencing the degree of the penalty imposed is the value of money or property that the defendant benefited from and/or was lost by the victim.
For tampering with a voting machine, the penalty is a first degree felony. This is a very serious penalty with a sentence between five to 99 years in a state prison and/or a fine of up to $10,000.
For online harassment, the charge is generally a third degree felony. However, if the crime was pretending to send an electronic message of any type from another person, hoping the recipient would believe that the other person authorized this message, with the intent to harm or defraud, the defendant may instead be charged with a "Class A" misdemeanor (not more than one year in a county jail and/or a fine of no more than $4,000). In the event, however, that this was intended to summon a response by emergency personnel, it will be elevated back to a third degree felony.

Bipolar Disorder is a brain disorder that causes unusual shifts in mood, energy, activity levels, and the ability to carry out day-to-day tasks.
